Permit For Food Handling Card

​Introduction

Food handler’s card is a document for those who are involved in food processing enterprise. All food handlers must have this card. To have this card, food handlers must undergo a medical examination, TY2 vaccination, a copy of his/her identity card or work permit and 2 identity card sized photographs

How To Apply

  1. The forms are available at the Department of Environmental Health at Level 17 and downloading can be done at the MBSA website.
  2. The completed forms must be submitted together with the required documents. ​

Guidelines

  1. Applicant and assistants are required to undergo medical examination and TY2 vaccination at any of the government or private clinics using the form issued by the Department of Environmental Health, MBSA only. 

  2. Food Handling Course Certificate (1 copy). 

    • Both applicant and assistant must attend the food handling course organized by the Panels appointed by the Department of Environmental Health, MBSA or training school recognized by the Ministry of Health. 
    • The appointed List of panels is shown in Appendix A. 
    • The fee for the Course is RM 50.00. 
    i. Every Friday. 
    ii.8:00 am - 12:00 noon. ​
  3. 3. 2 Identity Card-Sized photographs

  4. Identification Card (1 copy - Citizen) 

  5. Passport and Work Permit (1 Copy - Non-Citizen). 

  6. Expiry date of the permit must not be less than 6 months on the date of application for KPM. KPM application is only for work permit owners. It is restricted to the owners of restaurant and catering sector alone. 

  7. Payment Receipt of Food Handler’s Card. 

  8. Validity of Food Handler’s Card - By the expiry date of TY2 injection or the expiry date of the work permit.​

Terms of Payment 

Payment of Food Handling Permit Card has to be made immediately upon application.

The fee for each application is RM 30.00
