Introduction

8. Kiosk.

How To Apply  

1. The forms are available at the Main Counter level G, Counter Licensing Department on the 13th floor, Sungai Buloh Branch Office, Kota Kemuning Branch Office and Branch Office at Setia Alam. 
2. Completed forms must be submitted together with one (1) copy each of the related documents. 
3. Hawker Licence Application Form duly completed must be deposited at the Department of Licensing Counter at level 13. 

Guidelines

Attach the following documents:- 

1. Copy of Applicant's Identity Card
   
2. Copy of business registration (ROC/ ROB) (Jika ada)
   
3. A copy of the residence verification document (Assessment Tax Bill - for applicants who live in the Shah Alam area) or (Electricity/Water Bill - for applicants who live outside the Shah Alam area)
   
4. Copy of Death Certificate/Divorce Letter (For Widow Status)
   
5. Copy of the Verification Card for Persons with Disabilities (OKU) from the Department of Social Welfare (JKM)
6. Copy of Permission Letter/Rental Agreement (For Premises/Stalls/Markets not owned by MBSA)
7. Application Site Location Plan and Application Site Photograph (Front and Side View)​
8. Copy of Income Verification (Salary Slip - for those who work with an employer) or (Bank Statement - for those who are self-employed)
   
9. Copy of Vehicle Registration and PUSPAKOM Inspection Food Truck (for business applications Food Truck)
10. Application Process Payment Receipt (for office use)​

Terms andCo​nditions 

1. The applicant must be a Malaysian citizen.
2. Applicants must be Malaysian citizens aged 18 years to 65 years old who are eligible to be considered.
3. The site approval process is dependent on the availability of parcels / technical reports.
4. Priority will be given to the citizens of Shah Alam.
   
5. The selection is also based on the criteria or qualifications set by the Council. 
6. All applications will be processed based on the procedures established through the Meeting of the Committee to Review & Process License Applications on a monthly basis.
7. The application is deemed to be a failure if the applicant does not receive any response from the Council after three (3) months from the date the application is received.
   
8. Successful applications will be notified by letter.
9. For applications of temporary hawkers and motorised static hawkers, the unauthorized locations are at the junction of roads, parking lots, excursion roads, school fronts, main roads and back lanes.
10. The types of sales allowed for temporary hawker site applications and motorised static hawkers are packaged food and beverages only.
11. The types of sales that are NOT ALLOWED for temporary hawker sites and motorised static hawkers are fried food, sugarcane/coconut water, baked food, chicken/goat/duck and nasi campur.
12. Applicants who have a background of kmpaun/do not settle the door tax payment will not be considered.
    

Payment must be made on the day of the application. 

1. The license fee must be paid within 14 days of the date of the approval letter. ​​
2. Process payment must be made on the day of the license application.​​
