Licence for Swiftlet

Licence Application Swiftlet covers all areas under the supervision of the Shah Alam City Council.

The forms are available at the Main Counter level G, Counter Licensing Department on the 13th floor, Sungai Buloh Branch Office, Branch Office at Kota Kemuning and Branch Office at Setia Alam.
Completed forms must be submitted together with one (1) copy each of the required documents.
The duly completed Swiftlet Licence Application Form must be submitted to the counter Licensing Department on the 13th floor or in the Main Counter Level G.
